Introducción: Los tumores intramedulares son poco frecuentes y en su mayoría tienen un origen glial. La resonancia magnética, el microscopio de alta definición, el aspirador ultrasónico y el monitoreo electrofisiológico intraoperatorio han aumentado la seguridad de la cirugía de estas lesiones. La resección tumoral completa es actualmente el gold standard en el tratamiento. El objetivo del este trabajo es desarrollar los pasos para el manejo quirúrgico de los tumores intramedulares y presentar nuestra serie de casos a modo ilustrativo.<br />
Material y métodos: La técnica quirúrgica fue analizada en base a videos de nuestro archivo digital. La serie de pacientes se obtuvo a partir de un estudio retrospectivo que incluyó a los pacientes operados con diagnóstico de tumor intramedular en nuestra institución desde 2013 hasta 2017. El seguimiento promedio fue de 24 meses (rango 6-48). Para valorar la evolución clínica se utilizó la escala de McCormick. <br />
Resultados: Se detalla la técnica quirúrgica utilizada por los autores paso a paso. En un período de 4 años se operaron 21 pacientes, cuyo orden de distribución según informe anatomo-patologico fue: 14 ependimomas (67%), 2 astrocitomas, 2 hemangioblastomas, 1 subependimoma, 1 hamartoma lipomatoso y 1 hemangiopericitoma. En 13 de 14 pacientes con ependimomomas (93%) la exéresis fue total. <br />
Conclusión: Los tumores intramedulares continúan siendo una patología donde el tratamiento de elección es la exéresis quirúrgica. Se revelaron los detalles técnicos que consideramos importantes en el manejo quirúrgico de tumores intramedulares de diversa estirpe y se presentó una serie de pacientes operados con resultados satisfactorios. ]]></dcterms:abstract>
