Material y método. Catorce pacientes adultos con craniectomías mayores de 30 cm2 y colgajos pulsátiles fueron estudiados ecográficamente. La atención sefocalizó en los cambios morfológicos intracraneanos inducidos por la maniobra de Valsalva y la tos.
Resultados. Durante la maniobra de Valsalva y la tos el cerebro fue propulsado en bloque hacia la craniectomía. Movimientos notorios se observaron a nivel centroencefálico y estefenómeno se atribuyófundamentalmente al ascenso de LCR desde la columna y la distensión de las cisternas de la base. La magnitud de los cambios morfológicos fue proporcional al esfuerzo espiratorio y la presión intracraneana.
Conclusiones. La maniobra de Valsalva y la tos provocaron cambios morfológicos bruscos e importantes a nivel centroencefálico, A la luz de teorías recientes se discuten las eventuales consecuencias de esta movilidad]]>

    Descripción. El acceso a la región se puede llevar a cabo a través de un abordaje translaberíntico, retrosigmoideo o subtemporal. La decisión depende del examen clínico del paciente y sus estudios por imágenes. Elfresado del poro acústico es utilizado para la remoción del componente intracanalicular del tumor y para la identyicación y preservación del nervio facial. El bulbo yugular y el canalículo coclear por debajo, el seno petroso superior y la artería subarcuata por arriba y los conductos semicirculares posterior, superior, la crus que los intersecta, el conducto endolínfático y el saco endolinfático hacia lateral son estructuras en estrecha relación anatómica con la región. El conocimiento de su localización es esencial para la preservaciónfuncional del nervio facial y la audición.
    Conclusión. El conocimiento anatómico del área a fresar es elemental para alcanzar el resultado más cercano al standard contemporáneo de remoción total del tumor con preservación funcional del nervio facial y la audición, con la ayuda de una experimentada técnica microquírúrgica y monitoreo neurofisiológico intraoperatorio. ]]>

    Método. Desde junio de 2006 a diciembre de 2010, 7 pacientes fueron operados por presentar un EM.
    Resultados. De los 7 pacientes, 5 fueron mujeres y 2 hombres. El promedio de edadfue de 47,7 años, con un rango de 18 a 66 años. En cuanto al tamaño y la localización, la presentaciónfue muy variada. Desde menor a mayor en cuanto al tamaño, las localizaciones fueron: [1,3], [142-L3], IL 1-L21, [T11-L1], [T12-L2], [1,1-L4] y [L2-S2]. En 5 pacientes se realizó resección macroscópica total (71 % de los casos). En los 2 casos restantes, con resección subtotal, se realizó tratamiento con radioterapia luego de la cirugía.
    Conclusión. La resección micro quirúrgica total es el tratamiento de elección en los pacientes portadores de EM, siempre que la preservación de lafunción neurológica sea posible. Cuando la exéresis completa no es posible, y se realiza una resección es subtotal, está indicado realizar radioterapia como tratamiento adyuvante.]]>

    Presentación. Paciente de 73 años con trastornos en la marcha y paresia braquial derecha de tres semanas de evolución secundarios a hematoma subdural (HSD) frontotemporoparietal izquierdo.
    Intervención. Tras doble evacuación y craniectomía descompresiva por resangrado en postoperatorio inmediato. evolucionó con sensoriofluctuante y confuso, evidenciando en tomografías computadas (TAC) controles colecciones liquidas bilaterales extraaxiales con ventrículos, surcos y cisternas conservados, "signo de venas corticales" a través de cada colección con herniación de ésta a través de la craníectomía. Por punción transcutánea a través de ella se mide la presión del espacio subdural de 26 cm de H20 apoyando el diagnóstico de HEA. Se coloca drenaje subduroperitoneal bilateral logrando una evolución favorable.
    Conclusión. El manejo terapéutico óptimo de las colecciones subdurales aún no ha sido establecido. La conducta conservadora resulta adecuada en casos asintomáticos y sin efecto de masa mientras que en efusiones subdurates bastaría irrigar la cavidad o una derivación subduroperitoneal. Las derivaciones de LCR resultan de impredecible eficacia en HEA y las colecciones asociadas persisten o recidivan tras su evacuación, por lo que una correcta selección de pacientes es clave antes de cualquier decisión terapéutica. ]]>

    Descripción. Se presenta el caso de una joven de 20 años con Mucopolisacaridosis tipo VI (síndrome de Maroteaux-Lamy), en terapia de reemplazo enzimático de 3 años de evolución, que presentó síndrome del túnel carpiano bilateral y del túnel tarsiano derecho.
    Intervención. El diagnóstico se confirmó neurofísiológicamente y la descompresión quirúrgica brindó resultados clínicos muy favorables, mejorando notablemente la calidad de vida de la enferma.
    Conclusión. Esta rara patología y su asociación con las neuropatías compresivas debe ser conocida por neurocirujanos, ya que suele requerir tratamiento quirúrgico por la especialidad.
